    HOW TO ACCESS THIS SERVICE:

    1. You must call Member Services at the toll-free number listed in your membership materials before setting an appointment with a provider.
    2. We then call the provider on your behalf to verify your eligibility and to maximize and assure your savings.
    3. We call you back to provide you with the referral to the provider and all the pertinent details.
    4. You call the provider directly to schedule your appointment.
    5. You present your membership card upon arrival to your appointment with the provider and ask them to call Member Services at the toll-free number listed in your membership materials if they have any questions.
    6. The provider works up your bill based on the program fee schedule.
    7. If there are any problems with the provider recognizing the program rates, have the provider call Member Services at the toll-free number listed in your membership materials before you see the provider.
    8. You pay the provider in full at the time of treatment.
